NHS England have released a new assessment tool for modern general practice  – a diagnostic tool developed and tested with 540 practices across the country. It supports practices to review their care navigation and clinical triage process, and digital workflow, to identify what is working well and to identify and prioritise areas for improvement.

The comprehensive guide supports general practice teams to review and enhance their processes by offering a practical, step-by-step approach to assess, prioritise and allocate patients to help them reach the right care, first time. The guidance can be followed end to end to develop an entirely new care navigation process. Practices who  already have a process in place and want to improve or add certain elements can simply dip into the relevant sections. The guide includes access to editable resources to save practice staff time.
