The news is out and there is lots of speculation around the changes, as well as news the BMA’s GP Committee have voted to reject the changes. So this is what we know so far but subject to change …
Below we have summarised what we know but this is just top level information at this stage. The devil is, as always, in the detail and the detail (guidance & business rules/codes) have yet to be released:
Retired Indicators –
- CHD015 (33 points), CHD016 (14 points), DM012 (4 points), HF003 (6 points), HF006 (6 points), HYP008 (38 points), HYP009 (14 points), STIA014 (8 points), STIA015 (8 points), NDH002 (18 points)
- Most relate to BP targets as there are two new BP control indicators
- Total – 147 points
Increased Indicators –
- DM034 (was 4 points) & DM035 (was 2 points) – primary/secondary statin use in diabetes, both increased to 8 points each
Reduced Indicators –
- CHOL003 (was 38 points) – decreased for consistency with lipid lowering indicators, decreased to 20 points
New Indicators –
- CD001 – 41 points – new BP control, ≤ 79 without frailty (combining & replacing CHD & STIA BP indicators), threshold 40-90%
- CD002 – 20 points – new BP control, ≥ 79 without frailty (combining & replacing CHD & STIA BP indicators), threshold 40-90%
- DM037 – 10 points – new annual diabetes 8-care process, threshold 40-90%
- HF009 – 12 points – 4-pillar therapy in HF with reduced ejection fracture, threshold 20-50%
- OB004 – 5 points – referral to weight management programme for adults with obesity, threshold 10-30%
- OB005 – 13 points – shared decision-making & pharmacotherapy indicator for obesity, threshold 50-80%
- NDH003 – 20 points – gestational diabetes added, threshold 50-90%
Other changes to indicators –
- AF006 – upper threshold increased from 90% to 95%
- STIA007 – antiplatelet added (Ticagrelor)
- VI001, VI002, VI003 – addition of improvement threshold calculations to help practices, through no fault of their own, get some recognition of achievement. It is unlikely this will include any PCA’s.
There are also a number of important updates relating to core practice and contract changes. Many of you may have seen some of these reported in the news, even if you haven’t yet had time to review the official documents in detail.
