(following a shared decision-making conversation)
We are often asked questions around the patient choice pathway.
This PCA criteria requires a personal contact or discussion recorded in the patient record, noting the reason for the intervention being declined. This contact may be face-to-face, video or telephone contact between the patient and a health professional.
What does this include?
- Patients who decline a complete review (so patient has received 1 invite and they decline – informed dissent codes)
- Single-indicator declines such as blood pressure declined (more specific codes where they are available)
- Many practices get reception to call patients to come in for BP, the patient declines so they code BP declined
These should also be reviewed on an annual basis – if the patient declined last year, it cannot be assumed they are declining again. The exceptions to this are CS005 & CS006.
