The Royal College of General Practitioners has published the Fairer Practice Toolkit – a practical, evidence-informed resource to help general practices reduce health inequalities.
Practices identify their own focus patient group – those most at risk of poor access and outcomes locally – and choose actions relevant to their context. The toolkit draws on research including the Core20PLUS5 framework and is free to access.
