% patients with diabetes who have had the following care processes performed in the preceding 12 months: BMI, BP, HbA1c, cholesterol, smoking status, foot examination, albumin:creatinine ratio & eGFR creatinine.
This is an all or nothing indicator – all 8 processes must be coded and no single indicator exceptions apply to this indicator. This is an annual target, it doesn’t all have to be done in primary care although it is your responsibility to ensure it is all done and they can be recorded anytime throughout the year (not all on the same day).
Real-life example – diabetic register = 797
31 patients had an annual review (so far this year), 9 patients have 1 or more of the 8 care processes missing – 70% currently achieved (thresholds are 35-75%)
