This indicator is all about patients with both T1 and T2 diabetes receiving a bundle of care processes annually, to monitor risk factors and support early identification.
It is not about hitting targets in terms of measurements. For example, one of the care processes is BP, however, the BP target for diabetes is DM036. Another care process is HbA1c, however, the target for HbA1cis DM020.
There are no exceptions for DM037, you have to record all 8 care processes otherwise you will not achieve.
