This QOF indicator is looking at patients receiving all 8 care processes, it is not looking at measurements or targets. It is an all or nothing indicator, so if a patient is unable to provide you with a urine sample or they refuse a foot check, they will remain as an unachieved patient due to elements of the 8 care processes not being completed. For the patient to achieve on DM037 they must have all 8 care processes completed between 1st April 2026 and 31st March 2027, there are no PCA or exclusion codes available. Upper threshold is set at 75% so does allow for some patients who refuse.
BP targets still apply in DM036 and HbA1c targets apply in DM020/21.
